 We clarified histological and immunohistological features of resected intramucosal cancer case with lymph node metastasis(LNM)and examined the relationship between the histological heterogeneity and lymph node metastasis. The presence of ulcer, the size of tumor, and caner invation in the whole mucosal layer is siginificant predicting factors for lymph node metastasis. Immunohistochemically, the absence of Ki-67 localization and double layers structure of MAC5AC and MAC6 is also predicting factors of LNM. Our findings suggest that the absence of double layered differentiation in intramucosal lesion is the highest risk factor for lymph node metastasis. Signet ring cell carcinoma may develop lymph node metastasis more frequently compared to poor differentiated adenocarcinoma.
