FICEによる早期食道癌の存在・質的診断法

Endoscopic Diagnosis of Early Esophageal Cancer, Using the FICE System

要旨　早期食道癌の拾い上げ診断は，特殊内視鏡の発展によりヨード染色にとって代わりつつある．新しい分光内視鏡システムFICEでは，術者が自由にRGB波長の組み合わせを選択し，病変・非病変の色調コントラストを強調した画像を作ることができるのが最大の特徴である．そのために，必ずしも高倍率でなくてもその色調コントラストから血管パターンを観察することができ，質的診断・深達度診断が可能である．今後，さらなるRGB波長の組み合わせを検討することで，より簡便で正確な内視鏡診断が可能になることが期待される．

　Recently, spectral imaging systems are being developed for screening and detailed examination of GI tract cancers. This is a new diagnostic tool to replace the iodine staining method for screening early esophageal cancer. The FICE system can reconstruct high grade spectral images decomposed from ordinary endoscopic images by freely selecting three wavelengths, and they provide non-magnified images with high light intensity as well as magnified images, leading to endoscopic evaluation of macroscopic type, demarcation line and vertical invasion of esophageal cancer. FICE is expected to enable us to perform endoscopic diagnosis of early esophageal cancer more exactly and more simply.
