治療の立場からみた食道癌に対する食道造影検査の有用性

Usefulness of Esophagogram for the Treatment of Esophageal Cancer

要旨　治療の立場からみた食道造影検査の有用性は，まず，深達度診断と，次に食道全体をイメージに入れ，病巣の占居部位を評価しての治療法の決定にある．食道癌では進行度や占居部位により選択すべき治療法は大きく異なる．また，進行癌症例では，がん性狭窄の程度や深い潰瘍を有する症例での瘻孔形成の診断に欠かせない検査である．ステント挿入の際にも造影所見を評価しステント長や挿入位置の決定がなされる．今日，術前治療や高度進行症例では化学療法や化学放射線療法が適応されることも多く治療効果判定にも重要な役割を果たしている．食道癌治療における食道造影の有用性が多岐にわたる実例につき症例を呈示して解説した．

　The advantages of esophagography include the information it gives about tumor depth and the easy comprehension it provides about the tumor location in the esophagus. The treatment of esophageal cancer differs greatly according to the tumor depth and location. Esophagography is also useful for judging the stenotic grade due to cancer and detecting fistula formation from a deep ulcer. It also provides information on the length of stent required and the insertion site of the stent. It plays a big role in estimating the efficacy of treatment after chemotherapy or radiochemotherapy previous to an operation or in advanced cases. The various advantages of esophagography in the treatment of esophageal cancer were discussed, with examples employed.
