Crohn病の治療―最近の知見：文献 review

Current Drug Therapy for Crohn's Disease

要旨　Crohn病に対する治療は，日本では栄養療法が第一選択となっているが，欧米では薬物療法が主であり，今現在様々な薬物に対する臨床試験が進行中である．従来は5-ASA製剤，ステロイド，免疫抑制剤等がCrohn病の治療に主に使用されていたが，現在TNFを標的とした治療の研究が目覚しく進歩し，臨床応用されている．だが一方では，抗TNF-α抗体療法も，中和抗体の出現やコストの面等，様々な問題を残していると言える．ここでは，Crohn病に対する従来の治療薬，新しく実用化された治療薬，また現在研究段階の治療薬に分類し，主要な論文に基づいて，臨床試験成績と主な問題点について解説した．

　In Japan, diet therapy is considered to be the most important treatment for Crohn's disease. In Europe and North America, however, drug therapy is the main treatment and there are several ongoing trials for new drugs. Previously, 5-ASA derivatives, steroids, and immunosuppressants have been used. Recently there have been remarkable developments with TNF targeting medications. It is important to note however, that anti-TNF therapy is not without complications. Here we review the literature and discuss drug therapy for Crohn's disease including recently approved medications, and those currently under study.
