Surgically resected submucosal adenocarcinomas of the stomach were examined histopathologically to investigate the relationship between histological types and biological behavior. As a result, the incidence of permeation of vessels and lymph node metastasis in papillary adenocarcinoma were apparently higher than that in all the other histological types. In differentiated adenocarcinomas, particulary, carcinoma containing a larger proportion of papillary component, there was a higher incidence of permeation of vessels and lymph node metastasis. Furthermore, carcinomas with papillary component, irrespective of the amount of papillary component, showed higher incidence of permeation of vessels and lymph node metastasis than that of carcinomas with none. These results imply that papillary adenocarcinoma is a histological type with highly malignant potential, so we should be very cautious when considering the indication for endoscopic mucosal resection therapy for this type adenocarcinoma.
