要旨　1990年代後半から増加した本邦の内視鏡的胃瘻造設術件数は2005年では造設約10万件，交換約30万件に達している．造設方法には従来と大きな変化はみられていないが，偶発症発生減少のため，瘻孔周囲の胃壁固定が併設されるようになっている．適応では，恒久的な栄養瘻以外に，一時的な栄養瘻や減圧目的とされるものも増加してきた．栄養剤投与は逆流防止の観点から，固形化栄養の考えが導入されてきた．カテーテル交換にまつわる医療事故はここ数年，マスコミの報道で知られるとおりで，刑事告発される事例が増加している．発生頻度は低いが死亡例を含む重症合併症となるため，安全な交換の方法についての活発な議論がされている．

　PEG (percutaneous endoscopic gastrostomy) has been performed in 100,000 cases for new patients and in 300,000 cases for catheter exchanges in the year 2005, which shows an increase from the last half decade of 1990. Although, there is no major change in technical points, gastropexy has been added to avoid complications. Transient or decompressive gastrostomy has been increased when indicated. To resolve gastro-esophageal reflux, frozen or jellied nutrimen has been used. Complications involving catheter exchanges have occurred in a few cases, with serious results. Safe methods for catheter exchange are discussed.
