安全性・経済性からみた早期胃癌に対するESD―医療経済とトレーニング法

Endoscopic Submucosal Dissection from the Standpoint of the Safety and Cost Effectiveness

要旨　早期胃癌に対するESDは，2cm以上の病変でも一括切除が可能であり，根治性を保ちながら胃を温存できる治療法である．安全・確実に治療を行うためには，まず癌進展の正確な診断能力と，十分な内視鏡操作技術が必要である．さらに，ESDに関する知識の習得，シミュレーション，豚切除胃を用いたトレーニング，指導者のもとでの実際の治療といった一連の修練が必要である．ESDは素晴らしい治療手技であるが，現時点では保険適応になっておらず，EMRの診療報酬の中で処置具や薬剤を使用しなければならないため，財政上の負担を強いられている．患者のQOLや医療費の観点から，ESDによる治療のメリットは大きく，早期の保険診療認可が待たれる．また認可後に爆発的な実施件数の増加が見込まれるため，リスクマネージメントの観点からも，トレーニングシステムの構築が急務であると考えられる．

　Endoscopic submucosal dissection (ESD) is an epoch-making treatment for early gastric cancer, since it provides us reliable en bloc resection even if the tumors are larger than 2cm. Precise preoperative diagnosis especially of the tumor spread and the possibility for scope manipulation are essential for safe and reliable ESD. In addition, sufficient knowledge of ESD technique, animal model training, hands-on training and actual practice under the supervision of an expert in ESD technique are necessary if the necessary skills are to be acquired. ESD is at present not covered by health insurance, but we anticipate its approval, since QOL of the patient can be maintained and the technique has the potential to reduce overall medical costs.
