Risk Management for Therapeutic Endoscopy of the Digestive Tract

要旨　偶発症発生頻度が少なくない消化管内視鏡治療を行う場合には，常にリスクマネージメントのありかたを検討しておかなければならない．インフォームド・コンセントによって，その必要性と生じる危険性を具体的に患者側に説明して理解を得ておくこと，さらに術前に危険因子についての状況を把握し，偶発症対策を講じておくことは内視鏡治療時の常識である．日本消化器内視鏡学会の各種委員会から安全な内視鏡診療を推進するための指針が提唱されている．内視鏡診療のありかたの基本理念を盛り込んだうえで，各病院の事情を考慮した最もふさわしいマニュアルが構築されることが期待される．

　Risk management for therapeutic endoscopy of the digestive tract attempts to reduce preventable patient injuries and liability claims, and to limit the frequency and cost of claims that may otherwise arise. The risk management committee of the Japanese society of digestive endoscopy has proposed several guideline and/or parameters for practice. In this paper, brief comments, including informed consent, management of anticoagulation and antiplatelet therapy and so on, were discussed for the purpose of safety in endoscopic performance. We have several professional guidelines which are widely available, but, each endoscopist must create his or her own manual suited to the manner of procedure in each hospital.
