要旨　近年，医療技術の進歩は各領域において目覚しい．特に内視鏡検査においても同様である．精密かつ複雑な内視鏡を用いた検査，治療等などが行われる一方，患者の苦痛を軽減するために，鎮静薬，麻酔薬を使用するsedationの必要性も増加してきている．内視鏡検査を学んだ麻酔医の立場より，内視鏡検査時に用いられる代表的鎮静薬（midazolam・propofol・flunitrazepam）と，これからの使用が期待される鎮静薬（dexmedetomidine hydrochloride）の紹介およびそれらの薬物の安全な使用方法について解説する．

　Recent developments in gastrointestinal endoscopy have greatly expanded the diagnostic and therapeutic capabilities of gastroenterologists. Adequate pain tolerance is essential for successful completion of a safe procedure, especially in prolonged examinations. Conscious sedation is an established procedure for gastrointestinal endoscopy. The aim of this report, provided by an anesthesiologist with experience in endoscopic procedures, is to give an overview of the sedatives which are commonly chosen for conscious sedation and to introduce a new sedative, dexmedetomidine, an alpha-2 agonist.
