膨潤麻酔併用による腹腔鏡下経皮的腹膜外ヘルニア閉鎖術（膨潤LPEC法）の術後に妊娠・出産に至ったAYA世代女性鼠径部ヘルニアの1例

Bilateral inguinal hernias treated with laparoscopic percutaneous extraperitoneal hernia closure with tumescent local anesthesia（tumescent LPEC method） in an adolescent and young adult（AYA） female patient resulting in pregnancy and delivery : A case report

◆要旨：AYA世代の両側外鼠径ヘルニアに対し，膨潤麻酔併用腹腔鏡下経皮的腹膜外ヘルニア閉鎖術（膨潤LPEC法）を行った．術後半年から妊娠を許可し，術後1年3か月で妊娠，術後2年1か月で帝王切開にて出産した．術後4年8か月（出産から3年）を経過し，両側鼠径部ヘルニア術後の再発，感染，慢性神経疼痛を認めない．膨潤LPEC法はAYA世代の女性の鼠径部ヘルニアに対し，妊孕性を保持し，異物挿入のない，再発をきたさない修復法として施行可能である．

　Laparoscopic extraperitoneal hernia closure with tumescent anesthesia（tumescent LPEC repair） for bilateral external inguinal hernias in an adolescent and young adult（AYA） female patient was performed. Pregnancy was allowed from six months postoperatively ; the patient became pregnant at 1 year and 3 months postoperatively and gave birth by caesarean section at 2 years and 1 month postoperatively. Four years and eight months postoperatively（three years after delivery）, the patient has no evidence of recurrence, wound infection or chronic nerve pain after bilateral inguinal hernia surgery. Tumescent LPEC repair can be performed for inguinal hernias in women of the AYA generation as a fertility-preserving, foreign body-insertion-free and recurrence-free repair technique.
