眼筋型重症筋無力症を合併した蝶形骨洞側窩進展の内反性乳頭腫例

Sinonasal inverted papilloma of the lateral fossa of the sphenoid sinus complicated with ocular myasthenia gravis

はじめに

　鼻副鼻腔内反性乳頭腫（sinonasal inverted papilloma：SNIP）は良性腫瘍であるが，5〜20％の割合で再発を認め^1），悪性病変も伴う場合もあり，注意が必要である。

　上眼窩裂症候群は，動眼神経（Ⅲ），滑車神経（Ⅳ），外転神経（Ⅵ），三叉神経第1枝（Ⅴ₁）に障害が生じ，眼球運動障害や眼瞼下垂，三叉神経第1枝領域の知覚鈍麻の症状が出現した疾患の総称である。全身疾患に伴う症例も多く，初診時の診断の遅れが視力予後に影響する可能性があるため，的確な病因診断と同時に治療を進めていくことが重要である^2）。

　今回われわれは，上眼窩裂症候群様症状を示し，眼筋型重症筋無力症を併発した蝶形骨洞側窩に進展した内反性乳頭腫で，慎重な鑑別を要した1例を経験したので報告する。

　In this case report, we described a sinonasal inverted papilloma of the lateral fossa of the sphenoid sinus with ocular myasthenia gravis that required careful differentiation based on a review of the literature. A 70-year-old male, presented in September X with diplopia and a mass extending from the right nasal cavity into the lateral fossa of the sphenoid sinus. Superior orbital fissure syndrome was suspected, and right endoscopic sinus surgery（ESS）was scheduled. Complications of ocular myasthenia gravis were identified immediately before surgery. Pathological examination revealed an inverted papilloma. Subsequently, steroid treatment was initiated and the patient's symptoms including diplopia resolved.
