Colon Capsule Endoscopy

要旨●大腸カプセル内視鏡（CCE）は唯一，経口内服で行うことが可能な大腸検査法であり，大腸内視鏡が回盲部まで到達できなかった場合や，潰瘍性大腸炎などの器質的疾患で大腸内視鏡の実施が困難な場合に限り，2014年1月に保険承認された．2020年4月には各種疾患で身体的負担のかかる場合やX線学的に大腸過長症を有する慢性便秘症の場合まで保険適用が拡大されたが，限定的な保険適用や費用面，同時治療が不可能な点から広く普及するに至っていないのが現状である．しかし，疼痛を伴わない内服による内視鏡検査であること，放射線被曝がないことは利点である．本稿ではCCEの機種，保険適用，前処置，大腸腫瘍の診断について概説する．

　Colon capsule endoscopy（CCE）, only one modality of peroral colorectal examination, can be an alternative to colonoscopy and CT colonography in cases in which a previous colonoscopy was incomplete or for patients who are ineligible for colonoscopy due to inflammatory bowel diseases and other conditions. Furthermore, CCE indications of Japan's health insurance system have been extended to some physical distress and dolichocolon with chronic constipation. Limited indications, high cost, and unfeasibility of endoscopic treatment have prevented the wide use of CCE. This chapter summarizes the specifications, preparation, indications, and colorectal tumor detection of CCE.
