要旨●CTC(CT colonography)は,大腸を拡張して撮影したCTデータを3次元的に再構成し,病変を立体的に描出する画像診断法である.治療対象となる病変の検出は概ね可能であり,前処置の軽減や診療放射線技師による単独実施も可能となったことで,スクリーニング検査としての実用性がいっそう高まっている.さらに,早期癌の診断においても仮想注腸像やMPRを活用することで深達度の評価が可能となり,新たな画像診断法として今後の発展が期待される.
CT colonography(CTC), a three-dimensional CT imaging technique, allows stereoscopic visualization of the colon. With advances in bowel preparation and regulatory changes enabling radiological technologists independently perform examinations, CTC has gained increasing attention as a practical screening modality. It demonstrates high diagnostic accuracy for lesions measuring ≥6mm(sensitivity, 88% ; specificity, 92%)and constitutes a valuable alternative for fecal occult blood-positive patients avoiding colonoscopy. Furthermore, virtual barium enema views and multiplanar reconstruction enable direct evaluation of mural invasion and depth of early colorectal cancer. By integrating traditional morphological principles from barium studies with CT-based sectional analyses, CTC has evolved into a user-friendly and reproducible tool that bridges endoscopic and pathological perspectives for early colorectal cancer diagnosis.
