リンパ管造影により縮小が得られた後腹膜囊胞性腫瘤

Retroperitoneal cystic tumor that shrunk after lymphangiography

　症例は72歳男性．腹部超音波検査で下腹部に囊胞性腫瘤を認め当院紹介．腹部造影CT検査にて右後腹膜囊胞性腫瘤を認め，リンパ囊腫やリンパ管奇形が疑われた．腫瘤は右総腸骨動脈に接しており手術リスクは高いと判断し，経皮的ドレナージを施行．腫瘤は一時的に縮小したが，ドレナージチューブを抜去すると再増大を認め，診断治療目的にリンパ管造影を施行したところ腫瘤縮小を認めた．現在まで腫瘤縮小を維持している．

Abstract

The patient is a 72-year-old male. He was referred to our hospital because of a cystic mass in the lower abdomen on abdominal ultrasonography. Contrast-enhanced CT scan of the abdomen revealed a cystic mass in the right retroperitoneum, which was suspected to be a lymphocystic mass or lymphatic malformation. The mass was in contact with the right common iliac artery, and we judged that the risk of surgery was high, so percutaneous drainage was performed. The mass temporarily shrunk, but when the drainage tube was removed, it re-enlarged, and lymphangiography was performed for the purpose of diagnosis and treatment, and the mass was found to have shrunk. The mass has remained smaller up to the present (Rinsho Hinyokika 79 : 1097-1100, 2025).
