・左下腿後面に生じた異所性乳房外Paget病と考えた症例を経験した．

・病理組織学的には，異型性の強い腫瘍細胞が小型の充実胞巣状・管状様に浸潤増殖し，紅色局面部には典型的なPaget細胞が認められた．

・免疫組織学的には，CK7（＋），CAM5.2（＋），34βE-12（＋），Vimentin（＋），CK20（−），CEA（−），p63（−）．診断に苦慮したが，総合的に異所性乳房外Paget病と考えた．

A case of ectopic extramammary Paget’s disease of left lower leg

A 57 years-old man presented with ectopic extramammary Paget’s disease of left lower leg . The lesion was well-demarcated reddish plaque with a ulcer and necrosis lesion in central area. Histological evaluation of the epidermis revealed typical Paget’s cells, a tumor focus composed of adenocarcinomatous cells in the deep layer of the dermis. Immunohistological staining was positive for cytokeratin 7, CAM5.2, 34βE-12, Vimentin. He was finally diagnosed with ectopic extramammary Paget’s disease.
