 The low-density lipoprotein apheresis device Rheocarna Ⓡ is used to treat lower-extremity wounds in cases of occlusive atherosclerosis in which revascularization is not feasible. While it is important to recognize that Rheocarna is an adjunctive therapy and not a substitute for revascularization, its use can be a lifesaver for wounds showing no healing tendency due to poor peripheral blood flow.

 Rheocarna is also well-suited for treating superficial ulcers at the distal toe, but it is less appropriate for extensive lesions and those with uncontrolled infection.

 Before Rheocarna is administered, it is essential to confirm that the patient is not taking an oral angiotensin-converting enzyme (ACE) inhibitor. In addition, caution is required at the start of Rheocarna treatment regarding the potential presence of edema and increased infection risk due to improved peripheral circulation.
