形成外科領域におけるロボット手術の海外の現状と今後の日本の動向

Current Status of Robotic-Assisted Plastic Surgery in Japan

はじめに

　形成外科領域におけるロボット手術の海外での動きは，Robotic Assisted Microsurgical & Endoscopic Society（以下，RAMSES）がそのはじめと考えられる。

　この学会は，ロボット支援下マイクロサージャリーと内視鏡外科手術を進める学会である。2010年にその活動が，英国のロンドンで開始された。その後，年1回2日間の日程で国際学会が開催されている。著者が2015年7月にTexas Medical Center（米国，ヒューストン）を訪れた際，MD Anderson Cancer centerのJesse C. Selber教授を紹介された。その当時からSelber 教授は，形成外科領域のロボット支援下手術とマイクロサージャリーのトレーニングにおいて，多くの業績 1）2）をもち臨床を実施していた。その後，RAMSES 2018（コロンビア）とRAMSES 2019 （ブラジル，サンパウロ），そしてコロナの影響で2年間中止になり，2022年に再開し，RAMSES 2022（米国，フロリダ）に出席した経験から，形成外科におけるロボット支援下手術の海外の現状と日本における動向について報告する。

　The use of robotic-assisted surgery (RAS) has become common in many specialized areas, however, the field of plastic and reconstructive surgery (PRS) has seen little utilization of surgical robotics. We report on the current uses of robotics in PRS and consider future roles for robotics in this area. RAS has been approved for PRS in Japan and in various other countries. Almost half of the reports on RAS in PubMed research are on robotic mastectomy and breast reconstruction, including the use of implants and the latissimus dorsi muscle. We describe the history and current status of RAS in Japanese clinical fields, the current status of RAS in global PRS fields, and the future of RAS in PRS.
