はじめに

　動静脈奇形（arteriovenous malformation：以下，AVM）は進行性疾患であり，しばしば難治性である 1）。われわれは，AVMや巨大な静脈奇形に対して周囲二重結紮法という術式を開発し，治療を行っている 2）3）。計画する切除線の内外側により糸のナイロンで縫合結紮を行い，数cmの範囲で結紮を行った後にその間隙を電気メスで切離する。これを全周性に繰り返して，一塊に切除する方法である。腫瘤に切り込むことになっても出血は少量で済むが，重要な神経血管周囲の病変には適応は限られる（図1）。胸壁に発生したAVMの治療報告はまれであるが，いずれも難治性疾患として報告されている 4）～9）。周囲二重結紮法を施行し良好な結果が得られた胸壁AVMの1例について報告する。

　We report the case detail of a patient with an arteriovenous malformation (AVM) with a prominent chest wall protrusion that was treated with a circumferential parallel ligation method in which a needle-and-thread ligation was performed on both sides of the resection line. Although the patient (a 29-year-old female) required a blood transfusion due to relatively heavy bleeding around the ribs area, the postoperative course was satisfactory, and there was no tendency for the AVM to recur. We believe that the circumferential parallel ligation method applied in this patientʼs case is a useful surgical technique for AVMs when there is no significant neurovascular tissue in the vicinity.
