Re-irradiation for prostate cancer

局所限局前立腺癌（localized prostate cancer：LPC）に対する根治的放射線治療（radiation therapy：RT）後の局所再発に対する救済治療として局所療法の様々なモダリティについての報告がなされてきたものの，いずれも標準治療としては確立されておらず，全身療法としてホルモン療法（androgen deprivation therapy：ADT）が選択されることが多い。ADTは根治療法ではないために，いずれ去勢抵抗性再発に至る可能性があり，長期継続による様々な有害事象はQOLの低下につながりうる。再根治を目的とした局所療法として，従来は小線源治療（brachytherapy：BT）の報告が多かったものの，近年では局所焼灼療法とともに，画像誘導放射線治療（image-guided radiotherapy：IGRT）技術の進歩に伴い，体幹部定位放射線治療（stereotactic body radiation therapy：SBRT）を中心とした高精度外部照射技術による再照射の報告が増加する傾向にある。本稿ではLPCに対するRT後の再照射について概要を述べる。

Various local treatment modalities have been reported as salvage therapies for local recurrence after definitive radiation therapy for localized prostate cancer, but a standard treatment has yet to be established. Androgen deprivation therapy（ADT）is commonly selected as a systemic therapy in Japan. However, since ADT is not curative, it eventually leads to castration-resistant recurrence, and its long-term adverse effects can significantly impact quality of life. Historically, brachytherapy was the primary re-irradiation option for local recurrence, but recent advancements in image-guided radiotherapy（IGRT）have led to an increase in reports of re-irradiation using precise external beam techniques, particularly stereotactic body radiation therapy（SBRT）. This article outlines local salvage therapies such as ablative therapies and re-irradiation modalities for prostate cancer.
