Imaging diagnosis of gallbladder and biliary tract diseases

胆道疾患は，急性炎症性疾患や良性病変から悪性腫瘍まで多岐にわたり，画像診断が診断・治療方針決定において中心的な役割を担う。近年，超音波検査，CT，MRI/MRCPの技術的進歩とともに各疾患の画像所見の理解が深まり，診断精度が向上している。本稿では，急性胆嚢炎・急性胆管炎といった急性炎症性疾患，胆嚢ポリープ・胆嚢腺筋腫症・原発性硬化性胆管炎などの良性・炎症性疾患，さらに胆管内乳頭状腫瘍（intraductal papillary neoplasm of the bile duct：IPNB）から胆嚢癌・胆管癌に至る悪性疾患まで，2019年のWHO消化器腫瘍分類第5版をはじめとする近年の分類・概念のアップデートを反映しながら，各病態と各モダリティにおける特徴的画像所見を日常診療に即した実践的な鑑別診断の観点から概説する。

Imaging diagnosis plays a central role in the evaluation of gallbladder and biliary tract diseases, ranging from acute inflammatory conditions to benign lesions, precancerous lesions, and malignancies. This article reviews the pathophysiology and characteristic imaging findings of acute cholecystitis and cholangitis, benign conditions including cholesterol polyps, adenomyomatosis, and primary sclerosing cholangitis, precancerous lesions such as intraductal papillary neoplasm of the bile duct, and malignant tumors including gallbladder carcinoma and cholangiocarcinoma. Advances in ultrasound, CT, and MRI/MRCP have refined diagnostic accuracy, and practical approaches to differential diagnosis using multimodality imaging are discussed.
