Child abuse：diagnostic imaging and the role of radiologist

児童虐待は小児医療の中で主要な問題の1つとなっている。こども家庭庁発表の全国児童相談所の虐待相談件数でみると令和5（2023）年度で年間22万5,509件に及んでおり，救急で遭遇する可能性のある身体的虐待は5分の1強の22.9％を占める。このうち特に重篤な事例と考えられる警察が傷害・暴行などで検挙した件数は，警察庁の発表で年間1,903件に及ぶ。虐待による年間死亡数は確実性が高いとされるもので令和4（2022）年度には年間50件であり，同年度の小児の年間死亡数2,584人の1.9％にあたる。これは成人が殺人事件で死亡する率0.02％の実に100倍にあたり，看過できない問題である。

In addressing child abuse, the radiologist’s role is to perform their routine duties with precision. When physical injury is suspected, they should recommend appropriate imaging, supervise its execution, and apply suitable image processing. Findings must be objectively documented and shared with the referring physician. While differentiating other diseases, radiologists should indicate that child abuse may be a possible cause. By fulfilling these responsibilities, radiologists contribute to the detection and clinical assessment of abuse, providing both the opportunity and the evidentiary foundation for its recognition.
