3歳1か月男児。会陰部外傷を主訴に近医受診。腸管損傷が疑われ当院搬送された。KUB，CTで骨盤内に釘を確認したため，来院日当日に全身麻酔下で異物摘出を施行した。本症例では父親による児童虐待の可能性が疑われたため，児童相談所へ通告した。児童虐待が疑われた場合には，児童相談所への通告義務を負う。

A 3 year and one month boy was transported to our hospital because of perineal trauma and intestinal damage was suspected. We confirmed the nail in the pelvis by CT and KUB, which were removed under general anesthesia on the first day. Since we suspected the possibility of child abuse by his father, we notified this fact to the child guidance center. If child abuse is suspected, we are obliged to make a prompt notice to child consultation center.
