今回我々は，tree-in-budパターンを伴う膵管内乳頭粘液性腺癌（intraductal papillary mucinous carcinoma：IPMC）の多発肺転移の症例を経験した。症例は経気道的分布のCT画像を呈し，6年の長期経過で緩徐に増大していた。IPMCの肺転移の報告は検索の限りでは我が国で4例，英文で2例のみで，画像的に検討された報告はみられなかった。そこで，本症例について若干の文献的考察を含めて報告する。

A woman underwent surgery for intraductal papillary mucinous adenocarcinoma（IPMC）in her 60s. A CT showed slowly growing pulmonary metastases of IPMC over a postoperative course of six years. To the best of our search, there have been no imaging reports of IPMC pulmonary metastases with alveolar presentation. In this case, we hypothesize that these spread within the bronchial duct while producing mucus, resulting in an airway-centered distribution and slow progression.
