右胃大網動脈（gastroepiploic artery：GEA）グラフトを使用した冠動脈バイパス術（coronary artery bypass grafting：CABG）後のまれな合併症として，GEAグラフトが横隔膜を通過するために作成されるグラフト孔を介した横隔膜ヘルニアがある。今回我々は，CABG後8日目に診断された横隔膜ヘルニアの症例を経験したので，若干の文献的考察を加えて報告する。

A man in his 60s complained of nausea and vomiting after coronary artery bypass grafting（CABG）using the right gastroepiploic artery（GEA）. CT scans showed the herniation of the small intestine into the pericardial space through the hole in the diaphragm. Diaphragmatic hernia after CABG is a rare but potentially life-threatening complication which causes gastrointestinal and cardiovascular symptoms. The herniation into the pericardial space and the compression of the heart from anterior are characteristic findings of this condition.
