乳房超音波検査において，乳腺内病変による前方境界線の断裂は病変の皮下脂肪織への浸潤を示し，浸潤癌を強く示唆する所見である。今回，術前超音波検査で乳腺内の病変が前方境界線を越え，皮下脂肪織内に突出する像を認めたが，クーパー靱帯との位置関係からクーパー靱帯内に進展した乳管内病変と診断した乳癌の1例を経験したので，文献的考察を加え，報告する。

On breast ultrasonography, interruption of the anterior borders of the mammary gland shows the infiltration of breast cancer into subcutaneous adipose tissue, which indicates invasive breast cancer. We report a case of breast cancer in 40-year-old woman with an intraductal lesion extending into subcutaneous adipose tissue along the Cooper’s ligament. Ultrasonogram showed intramammary hypoechoic area protruding into the subcutaneous adipose tissue along Cooper’s ligament. Contrast-enhanced MRI showed the segmental non-mass enhancement lesion with nodular and linear enhancement into the subcutaneous adipose tissue. If breast cancers with the lesions protruding over the anterior border of the mammary gland along Cooper’s ligament on ultrasonography and MRI, we should recognize that the lesions may be intraductal components.
