今回我々は難治性のBasedow病治療経過中に撮像した¹²³I甲状腺シンチグラフィで，甲状腺両葉に多発する結節状の高集積を認め，Marine-Lenhart症候群が疑われた症例を経験した。しかしながら¹²³I甲状腺シンチグラフィで明らかな結節状集積を示すも，ドプラ超音波では結節内の血流がみられず，Basedow病治療に伴う腺腫様甲状腺腫と診断された。本症例は画像診断上のピットフォールを捉えた教育的症例であり，考察を交えて報告する。

We experienced a suspected case of Marine-Lenhart Syndrome whose thyroid scintigraphy showed multiple hot nodules in both thyroid lobes during the treatment of refractory Graves' disease. Despite obvious hot nodules in the thyroid scintigraphy, this case was diagnosed as adenomatous goiter with Graves' disease due to the findings in the thyroid ultrasonography, no blood flow inside of the nodules. Here we report this educational case, revealing the pitfall trapping at diagnostic imaging, with a literature review.
