WHOガイドライン改訂版から読み解く骨転移痛のマネジメント

WHO updated guidelines for the management of bone pain

がん患者が経験する様々な苦痛の中でも訴えが多く，がん診療に携わる医療従事者が基本的なマネジメントを知っておく必要があるのは「痛み」である。がん疼痛のガイドラインは国内外に各種発行されているが，その中でWHOのがん疼痛ガイドラインが2019年に改訂された。今回の改訂により名称が “WHO guidelines for the pharmacological and radiotherapeutic management of cancer pain in adults and adolescents” と変更され，evidence-basedを重視した内容に大きく変わっている。近年，日本ではメサドン，タペンタドール，ヒドロモルフォンと新規のオピオイド鎮痛薬が使用可能になり，がん疼痛の薬物療法はより複雑に様変わりしてきている。本稿ではWHOのガイドラインのアップデートにより大きく変更された点と，骨転移痛に対するビスホスホネート製剤および放射線治療に関する記述を中心に解説し，骨転移痛の薬物療法，および非薬物療法の理解を深めることを目的とする。

All health care providers who care patients with cancer have to know about basic management for cancer pain because most of the patients experience it. Of many guidelines for cancer pain, WHO guideline is the most famous and it was fully updated in 2019. The title of the guideline is “WHO guidelines for the pharmacological and radiotherapeutic management of cancer pain in adults and adolescents”, and the contents are described by evidence-based style. In Japan, pharmacotherapy for cancer pain have become complicated because more new analgesics including strong opioids methadone, tapentadol and hydromorphone have been available. The aim of this article is to understand the pharmacological and non-pharmacological management for cancer pain, through explaining the updated points in the guidelines and the descriptions regarding bisphosphonates and radiotherapy for bone pain.
