総論―前処置，検査法，画像作成，読影法などで注意すること

PETのもつ情報量はCTやMRIと比較すると圧倒的に少なく，画像から得られる情報は「集積があるかないか」，「集積が強いか弱いか」だけである。そこにPET/CTの場合にはCTから得られる解剖学的情報を加えて最終診断を下すことになるが，CTよりもPETの情報量が少ない分，多くの因子により画像が修飾されやすくなる。したがってPETの読影に際しては病変の集積の評価をする前に，その集積が本当に病変による集積かどうかを判断しなくてはならない。これには多くの画像に影響する因子やアーチファクトを知っておく必要がある。本稿では画像に影響を与えて診断能を低下させる要因や，それを防ぐ注意点について概説する。

Basically, volume of the data that composes PET image is relatively small compared to that of CT and MRI, and the information obtained from images is only “whether there is accumulation or not” “whether accumulation is strong or weak”. Because of small amount of information, PET image is easily affected by many other factors, such as status of pre-treatment, way of image acquisition, method of reconstruction, etc. Some medication or medical procedure also influence glucose metabolism to cause false negative/positive foci. Therefore, it is necessary to determine whether the accumulation is actual accumulation by lesions or false deposit made by other factors before interpretation of PET. To avoid a misinterpretation we have to know various factors and artifacts that affect the images. In this paper, I outline the factors that affect the image and reduce its diagnostic ability, and the precautions to prevent it.
