はじめに―FDG-PET/CTのややこしい症例の堤示（今月の症例から）

Forewords：pitfalls in image interpretation of FDG-PET/CT：case presentation from “Case of This Month” in Japanese Journal of Clinical Radiology

FDG-PET/CTは悪性腫瘍を中心とする診断の日常臨床において広く用いられ，その有用性も広く認識されている。FDGは悪性腫瘍だけでなくブドウ糖を使用する細胞全般に取り込まれるため，正常組織や活動性炎症病変，良性腫瘍などにおいても集積を認める。排泄される尿路系などへの集積もみられ，臨床的意義のない，もしくは重要ではない集積も画像上は多数描出される。前処置，データ収集から再構成画像を作成するまでの過程において技術的な問題で生ずるアーチファクトなどのピットフォールや，病変のサイズが小さい，細胞密度が低いなどの理由で集積が存在しても画像上認識されないという状況もあり得る。つまり偽陽性や偽陰性の問題と隣り合わせである。読影においても全身にわたりPET画像とCT画像を読影し，互いの関係を考慮しながら評価するため，見逃しや読み過ぎに注意する必要がある。そこで今回，“注意したいFDG-PET/CTでの見逃しと読み過ぎ” というタイトルの号内特集を企画した。

Uptake of FDG is observed not only in malignancies but also in benign lesion or even in normal tissue. Here, pitfall cases were presented from “Case of This Month” in Japanese Journal of Clinical Radiology. Some of them were difficult in image interpretation even for the experts of this modality. Review articles are organized in this section, which are useful for us to avoid pitfalls.
