A 73-year-old male patient presented with abnormal shadows in the left lung on chest X-ray. Detailed chest computed tomography (CT) revealed a mass in the left upper lobe. Bronchoscopy suggested non-small cell lung cancer. Systemic examination determined clinical stage T2bN0M0 (stageⅡA), leading to a decision for surgery. A planned thoracoscopic left upper lobectomy was scheduled. Intraoperative findings revealed calcified lymph nodes adherent to the pulmonary artery, necessitating conversion to open left upper lobe resection. Postoperatively, drainage volume was slightly elevated, leading to chest tube removal on day 10. The patient otherwise recovered uneventfully and was discharged on day 14. Postoperative pathology revealed predominantly adenocarcinoma components (pT2aN0M0 stageⅠB), with clusters of undifferentiated cells noted at the tumor margin. Undifferentiated components showed SMARCA4-deficient tumors. After consultation with the patient, adjuvant therapy was not pursued. Currently, one year and three months post-surgery, the patient remains disease-free.
