新型インフルエンザ発生時対応の実際と課題

Responses and Issues at the Time of the 2009 A(H1N1) Influenza Outbreak

　本調査の目的は、わが国で2009新型インフルエンザ（A/H1N1）の感染拡大した主な地域の保健所（行政機関）、医療機関、教育機関（小学校〜高等学校）における集団発生時の状況と対応の実際および課題を調査し、各機関の感染制御における看護活動の方策についての示唆を得ることである。個人情報および人権の保護のため兵庫県看護協会倫理委員会で倫理審査を受け承認を得た。調査項目は、①発生状況、②対応の実際と問題点あるいは課題、③マニュアルについて、④今後にむけての対策について、調査趣旨を説明して協力が得られた4保健機関、7教育機関、6医療機関から郵送あるいはヒヤリング調査を実施した。その結果、A/H1N1ウイルスが弱毒型であったため、いずれの機関においても、身体的問題よりも心理・社会的問題が大きいこと、集団発生防止のためには住民への啓発教育が重要であること、組織として柔軟に対応していくことが極めて重要であること、日頃からの教育や顔の見える関係づくりが必要であること等が強調された。

Abstract

　This study surveyed public health, medical and educational institutions in major 2009A (H1N1)influenza-affected areas in Japan to investigate the actual situation, actions and responses taken and problems encountered by those institutions at the time of the outbreak, and to gain suggestions regarding nursing involvement in infection control in each institution. The study was conducted with ethical approval and with informed consent from all participating institutions(4 public health, 7 educational, and 6 medical institutions)after a detailed explanation of the study's purpose. The survey was carried out either by postal questionnaires or interviews. Survey items included the infection situation as well as actions and responses taken and problems encountered. All institutions surveyed reported that since the A/H1N1 virus was of low virulence, more psychological and social problems were caused by A/H1N1 influenza than actual physical problems. The institutions surveyed also stated that to prevent outbreaks of influenza, it was essential to raise awareness of influenza among local communities, implement educational programs on a continuous basis, and build face-to-face relationships with all parties concerned.
