小児患者に対する持続的腎代替療法（CRRT）は，この20〜30年で大きな進歩を遂げ，日本国内でも多くの施設で実施されている。安全な小児CRRTには，装置・人員・知識・技術が必要であり，患者集約化が望まれるが，必ずしも容易ではない。本稿では，小児集中治療室（PICU）で対応する体重2kg以上の小児を対象としたCRRTに関し，小児急性血液浄化療法マニュアル第2版・日本急性血液浄化学会標準マニュアル（改訂第2版）での推奨および国内での現状をふまえたうえで，当科で実際に行っているプラクティスを述べる。本稿は，小児CRRTの経験がある，または普段小児患者は診ないが成人CRRTの経験が豊富な施設での，安全な小児CRRT実施の一助になることを目的とした。

CRRT for pediatric patients has made significant progress over the past two to three decades and is now performed in many centers in Japan. However, there are no established practices or guidelines for pediatric CRRT. For safe pediatric CRRT, it is desirable to perform CRRT in facilities with appropriate equipment, personnel, knowledge, and technology, but patient consolidation is not always easy. In this article, we introduce recommendations and the current situation in Japan regarding CRRT practice for pediatric patients weighing 2kg or more, and then describe the actual practice at our hospital. The purpose of this article is to help institutions that have little experience with pediatric CRRT, or that do not routinely see pediatric patients but have extensive experience with adult CRRT, to safely perform pediatric CRRT.
