Management of lower gastrointestinal symptoms：should constipation and diarrhea be prevented?

ICUにおいて便秘と下痢は頻繁に発生する日常的な問題である。しかし，研究で使用される便秘と下痢の基準には大きなばらつきがある。いくつかの観察研究で便秘や下痢は好ましくない患者アウトカムとの関連が示唆されているが，因果関係があるのかは不明である。予防的下剤投与や腸管管理プロトコルを評価したデータは限られ，現在のエビデンスからは，これらをルーチンに使用することは支持されない。便秘と下痢の基準を統一し，予防によりどの患者アウトカムの改善を期待すべきかを十分に考える必要がある。

Constipation and diarrhea develop frequently and are common problems in patients in the ICU. There is wide variability in the criteria for defining constipation and diarrhea in studies. Several observational studies have suggested that constipation and diarrhea are associated with unfavorable outcomes, but it is unclear whether there is a causal relationship. Data evaluating prophylactic interventions and bowel management protocols are limited, and current evidence does not support the routine use of prophylactic laxative administration or bowel management protocols to ameliorate these symptoms. The criteria for defining constipation and diarrhea should be standardized and careful consideration given to which outcomes are expected to improve with interventions.
