JR福知山線脱線事故は，典型的な交通事故による都市型災害の1つである。病院のICUの機能を維持するためには，病院前の搬送トリアージによる分散搬送を行う必要がある。また自施設のICUがキャパオーバーした場合には，安定した患者から他病院のICUに搬送することにより，自施設の診療と手術機能を維持しなければならない。また都市型災害の場合には，通常診察することが少ないクラッシュ症候群の集中治療管理を行わなければならないが，その管理は難渋することが多い。

The JR Fukuchiyama Line derailment was an urban disaster caused by a typical traffic accident. Decisions regarding transportation, triage, and distributed transportation were made in front of the hospital, with maintaining the function of the intensive care unit as a major goal. When the intensive care unit was unable to function appropriately because of overload, we transferred stable patients to other hospitals and maintained medical treatment and the surgery functions at the facility, especially for patients with crush syndrome.
