集中治療領域において気管挿管は必須の手技である。近年，直視型喉頭鏡以外にもビデオ喉頭鏡，軟性気管支鏡，ガムエラスティックブジーといった挿管器具が普及している。気管挿管において重要な点は，解剖と生理学の理解を前提に困難気道を評価し適切な挿管器具を準備・使用できることである。直視型喉頭鏡においてMiller型を用いるのはどのようなときか，ビデオ喉頭鏡は常に第一選択となり得るのか，軟性気管支鏡はビデオ喉頭鏡よりも成功率を上げるのか，ガムエラスティックブジーはスタイレットと比較し成功率を上げるのか，本稿では各挿管器具の利点・限界を中心に述べる。

Tracheal intubation is an essential procedure in the management of critically ill patients. In addition to direct-view laryngoscopes, intubation devices such as video laryngoscopes, flexible bronchoscopes, and gum elastic bougies have recently become widely available. The important point in tracheal intubation is to evaluate difficult airways, to prepare appropriate intubation devices based on an understanding of the anatomy and physiology, and to use these devices efficiently. When is it time to use a Miller-type in a direct-viewing laryngoscope?　Is the video laryngoscope always the first choice?　Is a flexible bronchoscope more successful than a video laryngoscope?　Does an elastic bougie have a higher success rate than a stylet?　This article focuses on the advantages and limitations of each intubation device.
