Prevention of ventilator-associated pneumonia (VAP) is important for early extubation. The primary mechanism of VAP is microaspiration of accumulated secretions around the endotracheal tube cuff. Although it has been reported that subglottic secretion suction (SSS) decreases the incidence of VAP, it is unknown whether SSS is cost-effective. To maintain the airway after extubation, it is important to evaluate aspiration risk and cough strength, but there is no established method. Prophylactic minitracheostomy to aspirate sputum has not been shown to prevent re-intubation and may delay the decision to re-intubate.
