たかがCT，されどCTである。本邦のCT保有台数は人口比でOECD諸国平均の4倍を超える。検査体制も広く整い，MRIや核医学と比べ，検査の敷居は低く，日常診療における第一選択として必要不可欠の役割を果たしている。しかし，簡便さゆえに「検査目的：スクリーニング」としか記載（期待）されていない場合もある。もったいない！　頭部CT，頸椎CTが発信する画像情報をくまなく適切に把握する重要性を，症例に即して述べ，診療への活用の一歩としたい。

Abstract

Although frequently regarded as a routine diagnostic modality, computed tomography (CT) has substantial clinical significance. As of 2024, Japan had 14,898 CT units, corresponding to 107 scanners per one million people, over four times the mean among OECD countries. With its well-established infrastructure and fewer accessibility barriers than magnetic resonance imaging or nuclear medicine, CT is a crucial first-line modality in daily clinical practice. However, its convenience occasionally leads to vaguely designated “screening” examinations without sufficient clinical context, representing suboptimal resource use. This review aims to highlight the importance of rigorous, contextually appropriate interpretation of head and cervical spine CT imaging and discuss strategies for its optimal integration into clinical decision-making.
