健康な若年女性の体位変換の循環器系への影響—脈波・コロトコフ音記録計を用いた観察

Influence of Postural Changes on the Circulatory System in Young Healthy Females: An Observation by Pulse Wave and Korotkoff Sound Recorder

はじめに

　体位変換時における循環動態への配慮は，長期療養患者の看護上，ショック状態等の異常の早期発見のためにきわめて重要である。看護学的立場からのこれまでの研究では，いずれも血圧，脈拍等を指標にした報告^1-3）や，心拍出量^4,5），血流量，血流速度等^6）の観察を行なった研究がみられる。しかし，血圧，脈拍に影響を与えている心拍出量，総末梢血管抵抗を含め，循環動態を詳細に分析した研究は必ずしも多くはない。そこで，長期就床患者に対する，より安全な体位変換法の確立のための基礎研究として，本研究では循環動態に関する血圧，心拍数はもちろん，心拍出最，総末梢血管抵抗，心係数，心筋負荷係数，心動作効率等の指標を取り上げ，若年の健康人の体位変換における循環動態の変化について観察をすすめた。

　This study intended to clarify the hemodynamic response induced by postural changes of subject saged 20 to 23 years old. Postures for the measurements were changed consecutively from supine to right recumbent, left recumbent, sedentary and erect position by their own volition. In order to exclude influence of postural change, after each movement the subjects examined were required to assume supine position for 5 minutes until subsequent measurements. The assesment of hemodynamic parameters at each position was performed immediately, 2 and 5 minutes after the rest by “Pulse Wave and Korotkoff Sound Recorder”.
