篩骨に発生した嚢胞形成性のcemento-ossifying fibromaの1例

Cystic Cemento-ossifying Fibroma of the Ethmoid Bone: Report of a Case

I．はじめに

　頭部・顔面領域に発生する骨形成を伴う線維腫性腫瘍はossifying fibroma，osteofibroma，osteoidfibromaなど種々の名称で呼ばれ，まだ混乱がみられる。しかしセメント質を含む腫瘍は歯根膜由来とされ，1971年のWHO分類^1）ではセメント質腫として一括され，その中に四つのサブグループを設けて分類，区別している。われわれは右節骨蜂巣に原発したセメント粒と骨形成を伴い嚢胞を形成したccrnento-ossifying fibromaを経験したが，これは上・下顎以外に発生することは非常に稀なものとされている。Krausenら^2）が篩骨に発生した本腫瘍の第1例目を報告し，その後Millerら^3），Cohnら^4）が報告しているが，本症例は篩骨領域に発生したcemcnto-ossifying fibromaとしては第4例目と考えられ，また嚢胞を形成した最初の症例と考えられるので報告する。

　A case of cystic cemento-ossifying fibroma of the ethmoid bone in a 14-year-old boy is reported.

　CT scanning disclosed a large cystic mass in the right ethmoid region which extended into the right orbit pushing the eyeball forward.

　The cyst was opened and the histological diagnosis of cyst wall was cemento-ossifying fibroma of the ethmoid bone.

　This is the fourth reported case of the cemento-ossifying fibroma of the ethmoid region and the first case of a cystic mass in this regin.
