頸静脈球高位について

High jugular Bulb: A Case Report and Review of Literature

I．緒言

　通常頸静脈球の上縁は鼓室底より低い位置に存在するが，まれに鼓室内に突出していることがある。このような症例は頸静脈球高位と呼ばれ，先天性血管走行異常の一つとされている。頸静脈球高位症例は，Page^1）が報告して以来外国ではいくつかの文献に報告例がみられるが，本邦ではいまだ纒った報告がないようである。

　最近筆者らは，滲出性中耳炎の診断で鼓室穿刺を行ったところ異常出血をきたし，鼓室試験開放にて鼓室底骨欠損を伴う頸静脈球高位を確認した症例を経験したので，外国における報告例と合わせて考察を加えた。

　High jugular bulb protruding into the middle ear space without bony covering is a rare entity and has been seldom reported. In a 59-year-old female with left tinnitus, the otoscopic examination showed red blue color in the lower half of her left ear drum, and profuse bleeding was encountered while exploratory puncture of the drum.

　Her middle ear was explored and revealed a red blue jugular bulb without bony covering in the lower half of the middle ear.

　Seven cases were found in the literature.
