I．はじめに

　近年耳性頭蓋内合併症は，抗生剤の発達により現在では，むしろまれな疾患とされているが，時には不十分な抗生剤の投与により病態が修飾され，以前よりも診断が困難となっているともいわれている。しかし最近CT scanなどの開発により早期診断が容易となり，治癒率も飛躍的に上昇しているものと考える。私どもは血性耳漏と意識障害を主訴として来院し，その臨床所見より耳性頭蓋内合併症を思わせた急性中耳炎と亜急性硬膜外血腫が併存した症例を経験したので報告する。

　A 13-year-old boy came to hospital with complaints of high fever, bloody aural discharge and confusion. Otogenic intracranial complication was suspected, and operation was performed. It was then revealed that there was a subacute epidural hematoma concurrent with acute suppurative otitis media. The clinical course, pathogenesis of otogenic epidural hematoma and prognosis in this case were described, and the literature was reviewed and discussed.
