I．はじめに
副腎皮質ホルモンの局所使用は，内服や注射に比べて副作用が少なく，その効果も早く耳鼻咽喉科領域においても多用されている。鼻疾患に対する本剤の局所使用としては，噴霧療法，下甲介注射法，点鼻療法などが知られている。点鼻療法が適応と考えられる疾患は，鼻炎（急性，慢性，肥厚性，アレルギー性，萎縮性），嗅覚障害，鼻茸および鼻科手術の後療法などである。鼻炎，嗅覚障害に対しては，その効果は非常に良く，有効率は70％以上を示すが，アレルギー性鼻炎や嗅覚障害に対しては，点鼻を中止すると症状が再発する例があり，長期投与が必要となる場合がある。副腎皮質ホルモンの点鼻療法において問題となるのは，その副作用である。数回の治療で治癒する場合は良いが，長期間使用を余儀なくされる場合は問題となる。そこで鼻アレルギーおよび嗅覚障害に対して副腎皮質ホルモン剤点鼻の長期使用例について，その副作用を検討したので報告する。
Nose drops of adrenocortical hormone were considered very useful for the treatment of nasal diseases, and we have used this treatment for many kinds of rhinitis, olfactory disturbance and nasal polyp.
Nearly 70% of the patients with nasal allergyand olfactory disturbance were recovered completely or satisfactorily by this treatment. But there were many cases that have necessarily used this nose drop for a long period of time. To know the side effects of this treatment, the examination with questionnaire and rapid ACTH test were performed, after administration of 5 ml/week of 0.1%dexamethasone or betamethasone solution for 12-36 weeks.
As the results, the minor side effects of adrenocortical hormone were recognized, but no major side effects were observed. Even after the administration of nose drops for 12 weeks the result of rapid ACTH test was normal.
