　I．緒言

　従来，方向交代性下向性頭位眼振は，一般に天幕上疾患や両側迷路が同時にしかも同程度に障害された時にみられるといわれているが^1）〜3），われわれは最近脳外科手術により，第1頸髄正中部腫瘍（神経鞘腫）と判明した症例において，本眼振を認めた。その手術所見から推定してこの眼振は，第1頸髄背側に生じた嚢胞の圧迫により両側延髄前庭神経核が，同時にしかも同程度に障害されたために出現したものと考えた。このように下部脳幹中心部障害によつて本眼振の発生をみた症例の報告はあるが，発生機序を考察した文献は見当らないので，ここに症例とその考察を報告する次第である。

　A direction-changing apogeotropic positional nystagmus in a 29 year-old female with a spinal cord tumor is reported. This particular type of nystagmus is generally known to be seen in the supratentorial lesion or in bilateral labyrinthine lesions occurring at the same time in same degrees. However, our case suggests another possibility that this nystagmus could have been the result of simultaneous involvement of bilateral vestibular system at the brain stem where they are topographically closely located.

　The authors postulated that the foregoing situation took place at the vestibular nuclei in the present case as the result of compression by the cystic mass (neurinoma) in the dorsal part of the spinal cord at the level of the first cervical vertebra, which was reveald by later surgery.
