めまい疾患と高脂血症

VERTIGO AND HYPERLIPOPROTEINEMIA

　I．はじめに

　最近数年来の大阪大学病院めまい外来受診患者の特色は，めまい・難聴・耳鳴のtriadの揃つたメニエール病確実例（厚生省メニエール病研究珍の診断の手引^10）による）が少なくなつてきたこと^{4）9）10）14）}と50歳以上の老年層の受診患者が多くなつてきたこと^6）7）9）である。大阪大学病院めまい外来の1969，1972，1974年のそれぞれの受診患者を比較した第1図からも分るごとく，1972，1974年の方に少しずつ，老年層の受診が多くなつてしる。これら老年層の受診患者には，脳内の何らかの循環障害すなわち椎骨脳底動脈循環不全症^3），脳動脈硬化症，糖尿病，高血圧症，低血圧症^13）（第2図参照）などの基礎疾患が前提―めまいの準備状態（素因あるいは発症因子^2））―となり，それに何らかの内的・外的因子がtriggerとして働くことにより起こるめまいが多く，その経過は意外に長いものがある。

　このように考えると，高脂血症，高コレステロール血症も脳循環障害によるめまいの立場よりみれば，めまいの準備状態の1つになる可能性がある。高脂血症にたとえば体位ないし頭位変換による血圧の一過性の変動がtriggerとして働き，脳循環障害をおこし，これがめまいの発症につながることは十分考えられる。

　During the period of two years from 1972 to 1973, measurements were taken on the serum protein of the vertigo patients who visited the vertigo clinic of the Osaka University Hospital. Following conclusions are made:

　1. During this period, hypercholesterolemia was found in 40% of vertigo patients. And among these patients more than 80% showed a high level cholesterol of borderline type.

　2. In cases of Ménière's disease 40% of the patients showed hypercholesterolemia of the borderline level. However, the presence of neither, triglyceride nor phospholid in high level was not met with.

　3. In cases of labyrinthine vertigo (corresponding to cases of suspected Ménière's disease), not only hypercholesterolemia but also triglyceride and phospholid of abnormally high level were found.

　4. In cases of labyrinthine vertigo and the vertigo due to hyperlipoprotein. who were referred to us from other physician, the hyperlipoprotein was observed, even, in the adolescent patients.

　5. Only a few hyperlipoproteinemia were observed in the patients with perceptive deafness, sudden deafness, congenital nystagmus and motion sickness.
