I．緒言
めまいを訴えて来院する症例は，近年次第に増加する傾向にある。めまいを来たす疾患は非常に多いが，これらの中で脳の血行異常，とくに椎骨脳底動脈血行不全によるめまいの出現頻度は高い3）12）18）。椎骨脳底動脈血行不全については，Kubikら（1946）8），Millikanら（1955）12），小川ら（1957）13）などによる臨床病理学的検討があり2）5），近年，脳血管撮影1）9）-11），ならびに平衡機能検査を主とした神経耳科学15）の発達により，これらを利用した研究も多くなりつつある。しかし，椎骨脳底動脈血流障害の際の椎骨動脈撮影所見と神経耳科学的所見とを対比検討した報告は少ない。
今回，著者らはめまいを訴えて来院し，脳血管写上明らかな椎骨脳底動脈の血流障害のみられた症例と末梢の後下，前下，上小脳動脈系の血流障害例について検索し，これら両者間の臨床症候学的，神経耳科学的差異について検討し，さらに2〜3の代表的症例を紹介する。
An attempt to establish a correlation between the neurological findings and vertebral angiographic findings is made by studying 20 cases of circulatory disturbances of the vertebral artery suffering from vertigo and dizziness.
1. Vertebral angiograms of all 20 cases showed circulatory disturbance. There were more than 50% stenosis in the extracranial vertebral artery in 5 cases; stenosis of the basilar artery, in 6 cases; and in the vertibrobasilar artery in another 8 cases.
2. There are no clinical signs by which the location of the stenosis or occulusion of the vertebrobasilar artery may be identified. But when the main branch of the extracranial vertebral artery becomes stenosed the symptoms become more prominent particularly, in hearing loss, tinnitus and other cerebellar signs.
