MYRINGITIS GRANULOSA

Ⅰ．緒言

　鼓膜特にその後上部から外耳にかけて，肉芽の発生を示す症例にはしばしば遭遇する。そのほとんどは，慢性中耳炎や中耳真珠腫に伴うものであつて，鼓膜に病変が限局し，中耳腔とは無関係な症例も少数ながら認められる。かような肉芽性鼓膜炎とみられる疾患は，古くからよく知られているが，もともと軽症で，手術的療法にいたらずして，治癒するのがほとんどのためか，その病態は，よく調べられているとはいいがたい。

　肉芽性鼓膜炎は，通常の外耳道炎に伴つておこり，その随伴症状としてみられる場合と，外耳道炎はなく，鼓膜にのみ肉芽形成をみる場合とがある。それゆえに，独立した鼓膜炎として本疾患をみる人と，外耳道炎として取り扱う人といろいろある。たとえばgranulating Myringitis（Moffett），Myringitis granulosa（Siirala），Otitis externa granulosa（Punt），granular external Otitis（Sadé），Otitis externa（Clark）などの呼び方が，文献に散見する。鼓膜は外耳道皮膚と異なつた解剖学的構造をもつているので，鼓膜に病変の主体がある場合には，鼓膜炎として取り扱つた方がすつきりすると私どもは考えている。

　Three cases of granulomatous myringitis considered as abnormal in type are reported. All three cases were operated upon and histopathological studies were made on specimens obtained from each. In one of them the growth was found to contain both, cholestrol crystals and giant cells while, others showed and ordinary type of granuloma. The granulomatous process was found to be involving the whole layer of the ear drum, and, particularly in one case, it was found to be invading the short Process or the incus. Based upon these findings, the authors suggest that, there might be a new type of granublomatous myringitis in which pathological involvements may be extensive in area.
