鼓膜麻醉時綿球挿入の一工夫

　通常鼓膜切開時に藥液貼布に依り鼓膜を麻醉せんとする際は，藥液を浸した小綿球を外耳道の深部に挿入し鼓膜面に壓挺せしめ，一定時間後に鑷子又は耳用鉤等にて該綿球を除去して鼓膜切開に着手するのである。併しながら鼓膜切開の對象である多くの場合の中耳炎時の鼓膜及び外耳道は極めて敏感であつて，些少の刺戟に依り激痛を訴える。患者が外耳道廣濶な成人の場合は綿球挿入除去共に極めて裝作容易にて豪も苦痛は無いが，外耳道狭小な婦人又は號泣喧噪靜止し難い子供等では之が困難を感じ，時には外耳道窪に綿球を迷入させたりして爾後の鼓膜切開施行に支障を來し，鼓膜麻醉の目的に對し却つて逆な現象をさえ招來するのは少からず經驗する所である。又裝作を容易にするため綿球を大にする時は藥液が不必要な個所に作用し，鼓膜自身には不充分な効果しかない事もある。

　私は斯る缺陥を除き，出來得る限り患者に苦痛を與えず且つ鼓膜麻醉の目的を充分達せしめるため下記の方法を工夫し，日常實施して好結果を得て居るので茲に紹介する。

MAKINO devises a method with which ap-plication of snesthetic agent in solution to tym-panic membrane for paracentesis is facilitated without causing undue irritation either, to the membrane or the external canal.

(1) A round cotton tampon made in size as to pass freely the external canal and threaded is saturated with the solution of anesthetic agent.

(2) Under an adequate vision of the field of operation through the otoscope and by means of ear probe this tampon is carried through the external canal to be pressed gentiy L ut firmly against the tympanic membrane.

(3) At the end of time allowed appropriately for anesthesia the tampon is gently removed by pulling on the attached thread.
