I．緒言

　頭蓋骨縫合早期癒合症の治療は，古くから可能な限りの早期外科的治療が良好な結果をもたらすといわれてきた．しかし，それと同時に手術術式そのものがその治療効果を左右するものとして各病態に応じて検討されてきた．近年，特にcoronal sutureからfronto-sphenoiclalそしてfrontoethmoidal sutureへとcranial vaultの縫合が頭蓋底の縫合に連続性をもち，前頭骨の発育不令をきたすcoronal sutureやmetopic sutureの早期癒合症においては，これらの前頭骨底の縫合の癒合症が存在し，cranial vauitのみならず前頭蓋底，眼窩，さらには顔面の変形，形成不金が生ずるという考え方が支配的となってきた．それに伴い，これらの縫合の早期癒合症に対する従来のstrip craniectomyの手術成績が反省され，近年欧米において，より根治的な手術術式が考案されてきた，しかしながら，本邦においてはこれらが広く普及しているとは言い難く，trigonocephalyにおける手術理論を前頭蓋窩の発育状態の分析から検討するとともに，その術式の選択につき，われわれが乳児期早期の根治術として行ってきたlateral canthal advancement・radical frontal remodelingの術式を示しつつ検討した．

The radical operative procedure for premature closure of metopic suture (trigonocephaly) was the-oretically discussed on the basis of the pathophy-siological concepts analyzed by the CT measurement. The authors experienced with 7 cases of trigono-cephaly of infancy during the last 3 years.
