I．はじめに

　近年，開頭術後に遅発性に出現する硬膜下膿瘍の報告が散見され，そのCT所見や臨床症状にかなりの特徴がみられている．また，外科的治療法としては，排膿，ドレナージ手術が主流を占め，大開頭による被膜外摘出術施行の報告は少ない．

　今回，われわれは，外傷による急性硬膜外血腫の術後8ヵ月の無症状期をおいて発症した硬膜下膿瘍の1症例を経験し，被膜外摘出にて良好な結果を得たので，主にその経時的CT所見と手術適応について若干の文献的考察を加えて報告する．

A case of encapsulated subdural empyema wasreported. This 1.5-year-old boy was admitted withthe increasing confusion and convulsion. Eightmonths prior to admission, he had craniotomy fortraumatic acute epidural hematoma on the left side.Following a coagulation of the middle meningealartery which was the bleeding source, the dura wasopened but no cortical damage was noted at that time.The computed tomographic (CT) scan on admissionrevealed a large subdural collection with a thin en-hancing rim on the left side.
