I．はじめに

　急性期脊髄損傷症例の観血的除圧術の適否についてはいまだ意見の一致をみない．損傷脊椎の支持機能再建の必要性は論を待たないが，損傷脊髄に対する積極的治療が，有効か否かで意見が分かれている．われわれは従来より，急性症例に対し積極的に除圧術を試みてきたが，その成績は満足するものとはいい難い．今回，観血的治療法として，さらに損傷脊髄の後方正中切截術（poste-rior longitudinal myelotomy）にて髄内減圧を行った急性期頸髄損傷例について分析したので報告する．

Althouth it is of great tragedy to loose motor andsensory function of the spinal cord by spinal cordinjury, there is no effective measure for complete cordlesions. In the central gray of the spinal cord at theregion of injury, hemorrhagic necrosis and edemaadvance causing secondary damage to the spinal cordin rather early stage after injury. It has not beenproved whether to remove necrotic tissue in the centralgray matter can be effective to prevent secondarydamage of the spinal cord or not.
